Mental Health Outreach Service

Living with a mental illness is a personal journey – and no two journeys are the same. We take a personal approach in supporting you to rebuild your momentum.

Our outreach services help people who are living with mental illness, increase your independence and participate in daily community activities. We help develop and re-establish daily living skills and self-care strategies, working towards you being able to integrate back into the community. These services are available on referral from Community Mental Health.

Encouraging your choice and decision making, we connect across services and a range of practitioners to support you to connect in your community. Working in collaboration with Community Mental Health case managers, Community Services, Housing NSW, drug and alcohol counsellors, Aboriginal Health Service, NSW Carers Association, as well as local doctors, psychologists and psychiatrists, we tailor our approach to help you achieve the best outcomes.
