If you live with complex needs, you might need support to care for your health.

NDIS Disability Health Supports is provided to participants who have complex individual support needs that are best met by someone with registered skills and experience in dealing with complex health issues.

Disability Health Supports can include assessment, planning, provision of treatment or care by and enrolled nurse or registered nurse.

Support we can offer

Momentum Collective is an NDIS Registered Therapeutic Supports Provider. We are registered to deliver disability health-related services such as:

  • Complex wound management
  • Subcutaneous injections
  • Management and care of urinary catheters
  • Management of bowel care
  • Stoma care
  • Management and maintenance of tracheostomy
  • Staff training

We have registered and experienced nurses who can provide nursing care to eligible NDIS participants.

When you get in touch with us, our nurses will access, plan, monitor, and review your health support plan including working with your GP or Health Care professional. Additionally, we will provide advice on how to engage in basic self-care and nursing, whereby one will become capable of performing simple tasks such as wound care or first aid on their own.

NDIS Support Categories

Nursing Supports is funded under Capacity Building Supports for disability health-related services in your NDIS plan and stated as a separate line item.

Please note: We bill in line with the NDIS Price Guide, which generally increases annually.
