Reaching out a helping hand
The Assertive Outreach program is a collective approach to improving homelessness. Our support team take a hands-on approach, working on the ground with those experiencing homelessness in the community. We build strong relationships founded on support and encouragement, equipping you to progress into long-term stable housing.
You will benefit from our team’s ongoing investment in a collaborative relationship designed to ensure you can meet your personal goals.
Our initial interventions are as brief as possible. We will then pass you the reins and take the supportive backseat. Benefit from a 12-month post-housing case management model that supports you to sustain your living situation. We will connect you with service providers and community assistance that will assist you in maintaining stability and independence.
Depending on your specific needs, we can assist you in:
- Gaining referrals to healthcare practitioners and support services
- Completing a NDIS application (where applicable)
- Linking you with additional outreach providers
- Learning about your tenancy responsibilities
- Advocating for your needs as your representative or support person at appointments.
You may have had an unexpected relationship breakdown, be struggling with an addictive substance, experienced a challenge to your mental health, or simply been unable to find another place at the end of a tenancy.
No matter the circumstances, you are not alone. Find your path back to permanent housing with us.
